Why families struggle with caregiver transitions
Talking about future caregiving means navigating difficult questions about changing roles, responsibilities, and expectations—conversations that many families struggle to initiate. Unspoken expectations and difficult emotions—particularly around siblings’ changing roles—delay these conversations until it’s too late.
As the longest-lasting relationship in the life of a person with a disability, siblings play an increasingly important role as parents age—yet rarely receive the support or preparation to participate in shaping the family’s future.
The heart of the Initiative
The Future Care Alignment Plan (FCAP) is the heart of the Initiative. Through facilitated workshops and activities, families build shared understanding, align expectations, and explore future caregiving together. The outcome is a living plan that evolves with life.
